The New York State/United University Professions Joint Labor-Management Committees (JLMC) address mutually identified issues affecting the quality and productivity of the State University of New York (SUNY) system.
NEW Fillable Online Evaluation Form
The Evaluation form is now an Online Fillable Form. You can access this form from any program. Simply open the evaluation form and fill it in as you did the application. Only the applicant’s signature is required. Generate the PDF form and submit it to the JLMC Committee at the address at the bottom of the form.
This evaluation must be submitted within 30 days after completion of each project or activity. Failure to submit a timely evaluation may result in your being ineligible for future funding from Joint Labor-Management Committee programs.
For Individual Development Awards, there is no requirement to submit a Program Evaluation to the JLMC. Each Campus Professional Development Committee determines the type of evaluation to be submitted to the Campus Professional Development Committee upon completion of the project or activity.

New Fillable Online Application
A new application form that combines the prior application and budget summary forms and is fillable online will simplify applying for JLMC program funds. To apply for funding from any Joint Labor Management Committee program, fill out the application online, print it, and obtain the required signatures. The process for submitting applications remains unchanged. For all programs except the Individual Development Awards, mail, fax or scan and email the application, with required attachments, to the JLMC staff. Applications and attachments for Individual Development Awards should be submitted to your campus Professional Development Committee.
